This guy is in a hospital bed - a large, swarthy, rough guy. I'm like an intern in the hospital, delivering something to his bedside. He's attended by a young blonde girl, apparently his wife, who is 'helping' him with something that appears to be intimate.

She says something about whether he would help her if she were in a similar situation, and I make a joke, but the conversation has turned all around. The man and I are joking that if he needed her help, this would be just the beginning of what she would be expected to do.
